We are always looking for people who can help us with current and future studies. If you are interested in participating in a study, please read the study agreement below and fill out the form that follows.

Current Studies:

• Pain after Shingles? Ages 18 and up
• Sleep Apnea and Depression? Ages 18 to 65
• Insomnia? Ages 18 to 85

Study Agreement

This is NOT a legal agreement. The following agreement is a confidentiality agreement between SomnoMedics and you.

This agreement confirms that all information is kept confidential, and is not released to any third party persons unless you authorize it. The information collected in the following form(s) is used only to apply for studies currently in progress at SomnoMedics, and/or future studies.

You do not have to fill in any area that you do not wish to, but a lack of information may negate you from participating in a study.Be completely honest on all of your answers. Any false information may also negate you from participating in current and future studies.
